Maybe one of these will help from the FMCSA.WE use one of these when we are in market for our advertising time in a certain area., and use the logsbooks for market to market. there are 4 forms to chose from.

I have mentioned this quite a number of time, Buckwheat in prior replys, I have a form that has to be filled out on a daily basis in order to prove both the mileage and the per diem.
