I just learned today the hard way that the DOT requires a new certification that the inspector is DOT certified.
Safety informs me that this new form is being included for forms requested after Jan 1,06. If you are like me you had/have forms on hand prior to this date. They wont work unless this new form is included.
To the best of my knowledge no general announcement of this change was made.
