Converting daycab to sleeper?

I have a 2003 Sterling straight truck, daycab. I'm wanting to add a single bunk sleeper to (36", 42", 48" whichever I can find). How would I go about getting the cab to bunk hole cut the same and also where could I get a boot to fit the two?

Do any of the sleeper companies (Double Eagle, ICT) do anything like this without having to purchase one of their sleepers?

If anyone has any advice I would greatly appreciate it!
