Fleet Owner Driver Contract (Van)

Does anyone have a template or copy of a contract that I can review and create a contract of my own. I am purchasing a new van and planning on putting a driver in my current van. Any help or advice would be appreciated.

Governing laws can vary, concerning contractor work, by state. When we added a driver about a month ago, I was referred to rocketlawyer.com. I had to add addendums for pay structure and insurance/truck requirements but we are happy with the ease of use. Check it out.
